A person-centered approach requires aligned agencies and systems for coordinated access to a comprehensive array of resources for physical, mental, social, and economic well-being. We have worked deeply within, and across, key social sectors that often share a common client population. We have a deep understanding and insight into the distinctive culture, services, and constraints of each sector—as well as what is common among them. Our perspectives allow us to translate between sectors and build bridges that align comprehensive resources around the people they mutually serve.
Community Development
We offer deep experience and knowledge to support communities challenged by historical social, health, and economic disparities in developing comprehensive, strength-based solutions that enrich neighborhood resources for economic development, housing, food access, transportation, culture, and recreation. Our team is versed in issues of holistic neighborhood revitalization, gentrification and displacement, and other community development concerns. Through national and local partnerships with community-based organizations, researchers, and practitioners we have a broad perspective on evidence-based and emerging practices for equitable community development grounded in community engagement and stakeholder engagement.
Early Childhood
As champions of maternal and infant health and early childhood development, we offer broad experience and deep expertise encompassing infant/child mental health integration into primary care, parenting training and support, early home visitation enhancements to address maternal substance abuse and depression, and access to high-quality early care and education. We have worked with state and local health departments, statewide coalitions, children’s hospitals, community health and social service providers, and neighborhood organizations to conduct research, facilitate stakeholder engagement and ‘lived experience’ perspectives, planning, and other support for a range of early childhood development initiatives in both urban and rural communities.
Healthcare
Our work with hundreds of healthcare agencies and coalitions over the past two decades has focused on integrated models of care that close equity gaps by addressing complex health physical and behavioral needs that incorporate the social drivers of health. We have worked extensively with hospital systems; primary care, behavioral health, and specialty care clinics; healthcare networks; healthcare payers; public health organizations, and social service, faith-based and advocacy community partners partnering with healthcare providers. We have delivered national, state, regional, and organization training for over 10,000 healthcare, public health, and social service professionals and students on improving healthcare services for people facing complex health and social needs.
Social Care
We have supported social service agencies at the system, organizational, and program levels for over two decades, delivering services to historically marginalized people and communities facing health and social disparities with a focus on low-income people and families of color. Our comprehensive services help organizations and coalitions develop integrated people- and family-centered services that support transformational change. In this work, we help our clients identify service-specific funding streams that can be blended and braided to support comprehensive service delivery models. Special populations served include people with complex physical and mental health needs, including mental health and substance abuse; formerly incarcerated people; pregnant people and families with young children; children from birth through adulthood facing challenges to housing, education, and employment; and older adults.
Justice
Our work in criminal justice and public safety spans the continuum of crime prevention, early intervention, services for the incarcerated support for people reentering the community after incarceration, and services for victims including domestic abuse and human trafficking. We have worked extensively in both the juvenile and adult justice systems, encompassing law enforcement, courts, and community-based initiatives, to strengthen law enforcement/public safety efforts. We have worked on public/private systems and program improvements for special populations within the justice and public safety systems, including people with unmet mental health needs, unhoused people, and victims and perpetrators of human trafficking and domestic violence.
